You will need to go to the main Police station at 611 W. 22nd Street to request a copy of a report.  To get a copy of the police report you must be listed in the report.  There is a charge of $7.50 per report. The records division is open Monday thru Friday, 8:00am to 12:00 and 1:00pm to 4:00pm.

You need to fill out a Property Release Request Form and then mail it to or drop it off at the main Police Station at 611 W. 22nd Street.
